Review existing information in patient record and examine orders and follow protocols for NPSG, Split, ASV and/or CPAP and BiPAP titration studies. Interview patient to assess current medications, recent caffeine, nicotine and/or alcohol ingestion, sleep pattern and need for treatment intervention during testing (i.e. insulin). Recognize chief sleep/wake complaint. Explain the study process and answer all patient’s questions and concerns. Set up and train patients about usage and maintenance of CPAP machines and masks when needed. Communicate with ordering physician or center/lab director to clarify unclear or inappropriate orders and communicate information about the patient’s status/results. Set up, calibrate and maintain cleanliness of monitoring equipment. Measure and apply appropriate EEG leads per 10/20 system, apply appropriate ECG, EOG and EMG monitoring electrodes. Place and secure recording transducers to measure airflow, respiratory effort and oximetry. Perform physiological calibrations, verify patient maneuvers and record. Implement “lights off” and note patient data. Monitor the polysomnogram, making notations every 20 minutes regarding sleep stage, body position, heart rate, biox and events noted. Note and correct equipment malfunctions to maintain quality study. Alter patient’s environment to obtain required sleep/wake data as needed. Monitor and change patient’s body position as needed to obtain required data. Perform and record calibrations, carefully wake patient and remove electrodes and transducers and assist patient with hygiene as needed. Summarize the polysomnogram and generate report summarizing sleep/wake period, sleep stages and events noted in each stage, REM episodes and any notations required per protocol. Recognize and respond appropriately to patient’s medical problems and physiological events. Maintain safe environment and ensure safety and welfare of all patients including those with special needs. Follow Standard Precautions regarding infection control. Apply and titrate CPAP therapy based on physician’s order and established protocols. Implement and document quality assurance activities to ensure optimum performance in all aspects of the sleep clinic. Maintain accurate records and file patient records.
